What is your Eligibility Status?

      • Full Time (FT) = 80 hours per pay period
      • Flexible Full Time (FFT) = 72 hours per pay period
      • Regular Part Time (RPT) = 60 hours per pay period
      • Part Time (PT) = 40 hours per pay period
      • Per Diem (PD) = as needed
      • ACA Eligible Status = those working on an annual average of 30 hours per week

Eligibility Waiting Period

Employees that start work with NMC in the first calendar week of a month will become eligible for benefits on the first day of the month following the month of their start date.  New employees who begin work on the 8th day of the month or later will become eligible for benefits on the first day of the second month after the month of their start date.

Health Insurance – CBA Blue

NMC offers three health plan options, Gold, Silver and Bronze, with different levels of coverage to meet you and your family needs. This is a preferred provider type plan. Participants may choose health care providers from BlueCross Blue Shield’s extensive network of providers. Employees who utilize NMC providers and services may have lower out of pocket costs. Eligibility: Full Time, Flexible Full Time, Regular Part Time, Part Time and ACA Eligible

HealthyÜ Wellness Program

Our HealthyU wellness programs works to inspire and empower employees and their families to build healthier lives.  We provide an array of activities and services to help you maintain or develop a healthy lifestyle you can be proud of. Dental Insurance – Delta Dental

NMC offers three dental plan options, Enhanced, Standard and Basic, with different levels of coverage to meet you and your family needs. Preventative care, including exams, cleanings, x-rays, restorative care including fillings, extractions, and root canals, major care including crowns, on-lays, dentures and orthodontia. This is a preferred provider type plan. Participants may choose providers from Delta Dentals extensive network of providers. Eligibility: Full Time, Flexible, Full Time, Regular, Part Time, and Part Time

Vision Benefits – Vision Services Plan (VSP)

Eye exams, prescription glasses, including frame and lenses, contact lenses, plus savings on Laser Vision Correction. Eligibility: Full Time, Flexible Full Time, Regular Part Time, and Part Time

Basic Life & AD&D Insurance

100% NMC funded for 1 times annual salary up to a maximum of $500,000 Eligibility: Full Time, Flexible Full Time, Regular, and Part Time

Voluntary Short-Term Disability Insurance

100% employee funded. Benefit begins on the 8th day after the occurrence of a qualified disability, benefit is 60% of current base rate of pay. Eligibility: Full Time, Flexible Full Time, Regular Part Time and Part Time employees. Eligible employees may also purchase plans for select family members.

Long-Term Disability Insurance

100% NMC funded. Coverage is effective on the first of the month following 90 days of service, benefit is 60% of monthly pay. Eligibility: Full Time, Flexible Full Time, Regular Part Time & Part Time

Voluntary Accident Coverage

100% employee funded. Pays benefits to members who enroll in coverage for on & off the job accidents, coverage can be chosen for an employee, spouse & dependent children. Eligibility: Full Time, Flexible Full Time, Regular Part Time & Part Time

Voluntary Critical Illness Coverage

100% employee funded. Pays benefits to members who enroll in coverage should a member develop a Critical Illness. Some of the Critical Illnesses include: Cancer, Heart Attack, Major Organ Failure, and Blindness. Eligibility: Full Time, Flexible Full Time, Regular Part Time & Part Time

Voluntary Pet Insurance

Tax Sheltered Annuity403(b) & Defined Contribution Pension Plan 401(a)

NMC Employees have a retirement program which combines both a 403(b) and a Defined Contribution Plan.  

Tax Sheltered Annuity 403(b)

  • NMC Currently matches up to 2% of an employee’s gross annual pay for employees who contribute at least 4% of their gross annual pay.
  • Eligibility: employees are eligible to participate in the 403(b) upon hire.

Defined Contribution Pension Plan 401(a)

  • NMC contributes 3% of gross annual pay for all employees who work at least 1,000 hours during a calendar year automatically.
  • This benefit is subject to a step vesting schedule

Northwestern Occupational Health 401(k) Plan

  • NOH matches up to 3% of employee’s gross annual pay for employees who contribute at least 3% of their gross pay.
  • NOH will contribute 2% automatically to all employees who work at least 1,000 hours in a calendar year & is subject to a step vesting schedule
  • Eligibility: employees are eligible to participate in the 401(k) upon hire.
