Welcome to Population Health Services
The Population Health Department at Northwestern Medical Center (NMC) sits at the intersection of clinical care, preventative care, and community support services to improve the lives of our friends, neighbors, and family members. We work to support the Community Health Team across multiple care delivery sites in our region.
We are a team that creates linkages for policy makers, providers, and community partners about health equity, the pathways between primary prevention strategies and the treatment of disease.
We offer education and workshops (online and in person) to individuals, businesses, and schools about vaping and quit strategies, chronic disease management, diabetes management, chronic pain management, and hypertension.

Health equity
Equitable access to care means you are receiving the right care, in the right place, at the right time no matter who you are or what your care needs are.
Preventative screenings for cancer
The Northwestern Medical Center team is actively working with our partners and throughout our community to decrease the impact of cancer.
Transitions of care
The Population Health Team is working with the hospital, agencies, and primary care providers to ensure health care transitions are safe, effective, and clearly communicated.
