NMC’s Patient and Family Advisory Council (PFAC) met in October and learned about the Care Partner Framework from Manager of Care Management Amanda Wilson.
The discussion included information related to:
• How the model can be used with same-day surgery patients to support them when they go home
• How NMC staff become aware that the Care Partner is onsite
• What happens when a patient does not have a Care Partner
• How communication happens with Care Partners who cannot physically be onsite
The PFAC first that began meeting in August of 2022 and meets monthly. The establishment of the group is a key action in the hospital’s Quality Plan to strengthen communication and collaboration between patients, their families, providers and hospital staff.
