Patient Centered Medical Home

The Patient-Centered Medical Home (PCMH) is a model of care that focuses on the patient. PCMHs build better relationships between patients and their clinical care teams. Research shows that PCMHs improve quality and the patient experience, and increase staff satisfaction—while reducing health care costs. Practices that earn recognition have made a commitment to continuous quality improvement and a patient-centered approach to care.

NMC is the administrative entity for the Vermont Blueprint for Health in the St. Albans Health Service Area. NMC oversees the administration and quality of the PCMH model, supporting our primary care partners and fostering relationships and learning of the Community Health Team.

Through partnerships we have an expanded Community Health Team that consists of:

• RN Care Managers
• Behavioral Health Clinicians
• Community Health Workers
• MAT Care Coordinators
• Licensed Alcohol and Drug Counselors

This team is embedded within our primary care, specialty care, long-term care, and home health agencies.
