The Patient-Centered Medical Home (PCMH) is a model of care that focuses on the patient. PCMHs build better relationships between patients and their clinical care teams. Research shows that PCMHs improve quality and the patient experience, and increase staff satisfaction—while reducing health care costs. NMC is the administrative entity for the Vermont Blueprint for Health in the St. Albans Health Service Area. NMC oversees the administration and quality of the PCMH model, supporting our primary care partners and fostering relationships and learning of the Community Health Team.

This team is embedded within our primary care, specialty care, long-term care, and home health agencies.

Medically Assisted Treatment Teams
Vermont initiated the Hub & Spoke system to support individuals struggling with opiate use disorder over a decade ago. This system provides medically assisted treatment in primary care and specialty service sites. Each site has a Medically Assisted Treatment Registered Nurse and/or Licensed Alcohol & Drug Counselor that provides ongoing support to patients.
