What is a Medical Home?

The Patient-Centered Medical Home (PCMH) is a transition away from a model of symptoms and illness based upon episodes of care toward a system of comprehensive coordinated primary care for children, youth, and adults.

Patient centeredness refers to an ongoing, active partnership with a personal primary care provider who leads a team of professionals dedicated to providing proactive, preventive and chronic care management through all stages of life.

These care teams are responsible for coordinating care across all health care systems to ensure patients receive care when and where they need it.

With a commitment to continuous quality improvement, care teams utilize evidence-based medicine and clinical decision support tools to guide care decisions.

Medical homes ensure that patients have the education and support to actively participate in their own care.

NMC offers telemedicine visits for a variety of services, providing high-quality care from the providers you know and trust.

How to schedule a telemedicine appointment: It’s easy to set up. When scheduling an appointment with us, we will let you know if a telemedicine visit is an option for the treatment you are seeking. If it is and you’d like to schedule a telemedicine visit, we will then give you instructions on how to make the connection, which will be via telephone, through a free video conferencing application, or by using NMC’s Patient Portal.
