Welcome to Monarch Maples Pediatrics

Primary Care Health Partners is thrilled to announce the addition of Monarch Maples Pediatrics located in Enosburg Falls and St. Albans, effective January 1, 2022. PCHP is excited to welcome Drs. Deanne Haag, Roya Mansoorani, Stacy Strouse, Robert Tyson and Tracy Tyson. The physical locations and phone number of our offices will remain the same. Phone Hours

Striving to be here when you need us, we take your calls early in the day, and through the lunch hour. You can reach a triage nurse starting at 8 a.m., an hour before our offices open. You can also speak to a triage nurse when you call between noon and 1 p.m. as well. 

Evening & Weekend Hours

In addition to our regular 9 a.m. to 4:30 p.m. office hours, our St. Albans office is open each weekday from 5-7 p.m. and on Saturdays and Sundays from 10 a.m. to 3 p.m. for urgent, sick appointments.

Integrated Behavioral Health

Behavioral Health Integration is care delivery in which the patient can receive both their physical health care and their mental health care in the same setting. 

Did you know that your Primary Care Provider’s office and Northwestern OB/GYN have embedded behavioral health social workers available to patients?  

  • Northwestern Medical Group (NMG), Northwestern Counseling & Support Services (NCSS) and Vermont Blueprint for Health partner to bring our community members immediate access to mental health and care coordination services directly in Primary Care and Northwestern OB/GYN. 
  • There are many factors that may impact an individual’s ability to access mental health care. Our team strives to reduce stigma, increase knowledge around whole-body wellness, and reduce potential barriers.   
  • Available for in the moment consultation, assessment and treatment at your next provider visit as well as ongoing support as needed.

NMC offers telemedicine visits for a variety of services, providing high-quality care from the providers you know and trust.

How to schedule a telemedicine appointment: It’s easy to set up. When scheduling an appointment with us, we will let you know if a telemedicine visit is an option for the treatment you are seeking. If it is and you’d like to schedule a telemedicine visit, we will then give you instructions on how to make the connection, which will be via telephone, through a free video conferencing application, or by using NMC’s Patient Portal.
