Some individuals may benefit from the individualized attention they can receive in our Lifestyle Medicine Clinic. Participants meet with our Medical Director, Dr. Elisabeth Fontaine to address health issues such as high blood pressure, high cholesterol or other elevated risk factors. Clinic staff includes wellness coaches, dietitians, chronic disease nurses, exercise specialists and support staff who use a multi-faceted approach tailored to each participant’s needs. The team tracks progress, provides encouragement and adjust strategies based on ongoing feedback.
When you are ready to quit for good, the right support can make all the difference. We are here to help you choose the option that works best, including:

Diabetes Management, Education and Prevention Our Certified Diabetes Educators work with individuals who are at risk of or have been diagnosed with diabetes. We are equipped to answer questions about diet, exercise, medications, blood sugar monitoring and aid with stress management. Individual action plans are developed and can be supported on an ongoing basis.
Medical Nutrition Therapy Our registered dietitians can help you develop a realistic plan to help prevent and/or manage a healthy weight or a strategic diet for your chronic condition. With specialized training in the roles of food and nutrition, our Nutritional Therapies include:
