Department:Chronic Disease
Position Description: Provides social work services in inpatient and outpatient settings. Takes lead in planning for effective discharge of patients from inpatient to outpatient and home settings.
Status/Schedule:

Shift Time Schedule:8:00 a.m. - 4:30 p.m.Average Weekly Hours:24 hrs/wk
Days:No on-call duty required. No weekends required.
Salary Range:Minimum:$19.55Midpoint:Maximum:$21.51
The above wage range reflects the minimum and maximum hiring range for this position.

    Qualifications:
    Education:Bachelors degree in social work required. Masters degree in social work preferred.
    Experience:Previous experience in health care required. Two years experience in health care field desired.
    Special Skills:Excellent verbal and written communication skills required.
