Position Title: RN Case Manager #10-00

Department:Chronic Disease
Position Description: Assesses appropriateness of intensity of services for patients, making recommendations for change, as needed. Applies utilization review criteria to daily assessments of patients needs. Supports medical staff in securing appropriate level of care for patients. Participates in discharge planning process to ensure continuity of care in the most appropriate setting.

Status:Flexible Full TimeShift:Day
Shift Time Schedule:Shifts: 7:30 a.m. - 4:00 p.m. and Saturdays 8:00 a.m. - 12:00 p.m.Average Weekly Hours:36-39 hrs/wk
Days:Weekends required as needed. No on-call duty required.
The above wage range reflects the minimum and maximum hiring range for this position.

    Qualifications:
    Education:Graduate of an accredited school of nursing required. Bachelor of Science Degree in Nursing required. Certification in utilization management is expected within four years of employment.
    Experience: Must have three years prior experience in a hospital setting and utilization management and/or case management experience desired.
    Special Skills:Must have excellent verbal and written communication skills.
    License Required:Current licensure as a Registered Nurse in the State of Vermont required.
