What’s your status? This will help you determine your benefit eligibility…
Full Time (FT) = 80 hours per pay period;
Flexible Full Time (FFT) = 72 hours per pay period
Regular Part Time (RPT) = 64 hours per pay period
Part Time (PT) = 40 hours per pay period
Regular Per Diem (RPD) = at least 8 hours per pay period
Per Diem (PD) = as needed
Health Insurance - MVP
Eligibility: Full Time, Flexible Full Time, Regular Part Time and Part Time employees.
This is a preferred provider type plan. Participants may choose health care providers from MVP’s extensive network of providers, or they may choose out of network providers, in which case the level of benefits will be lower. There is no requirement to choose a primary care provider, and employees may access specialty care without the necessity of referrals. A higher level of benefits is provided when enrollees use Northwestern Medical Center for inpatient or pharmacy services. Eligible employees may join the plan on the first of the month following one full month of employment.
Dental Insurance – Delta Dental
Eligibility: Full Time, Flexible Full Time, Regular Part Time and Part Time employees.
Preventive care, including exams, cleanings, x-rays; restorative care including fillings, extractions, and root canals; major care including crowns, onlays and dentures; orthodontia; subject to deductibles and co-payments. Eligible employees may join the plan on the first of the month following three full months of employment.
Vision Benefit – Vision Services Plan (VSP)
Eligibility: Full Time, Flexible Full Time, Regular Part Time and Part Time employees.
Eye exams, prescription glasses’, including frames, and lenses, contact lenses, plus savings on Laser Vision Correction. Eligible employees may join the plan on the first of the month following three full months of employment.
Life Insurance
Eligibility: Full Time, Flexible Full Time and Regular Part Time employees.
Paid entirely by the hospital; covers the employee only. Coverage equals one times annual salary. Additional optional insurance, including spouse & dependent coverage, may be purchased by payroll deduction. Eligible employees may join the plan on the first of the month following three full months of employment.
Long Term Care
Eligibility: Full Time, Flexible Full Time, Regular Part Time and Part Time employees. Eligible employees may also purchase plans for select family members.
Offered through John Hancock, NMC’s Long Term Care insurance plan allows employees to purchase coverage for long term care in the event personal care or supervision is needed for an extended period of time. Often referred to as ‘portfolio insurance’ because the cost of long term care has the potential to deplete an individual’s retirement funds. This plan is offered with various options to allow for a customized benefit for individual needs.
Pension Plan
Northwestern Medical Center contributes 3% of gross annual salary (subject to max limits) for all employees who work at least 1,000 hours during a calendar year.
Tax Sheltered Annuity 403(b)
Subject to annual review, Northwestern Medical Center currently contributes up to 2% of gross annual pay in matching funds for employees who contribute at least 4% and who work at least 1,000 hours in a calendar year.
Earned Time
Eligibility: Full Time, Flexible Full Time, Regular Part Time and Part Time employees.
Paid time off is to be used for illness, vacations, and holidays.
Exempt (salaried full time) - 31 days/year
Full Time Employees - 26 days/year
Flexible Full Time Employees - 23 days/year
Regular Part Time Employees - 20 days/year
Part Time Employees - prorated by hours worked
Short Term Disability
Eligibility: Full Time, Flexible Full Time and Regular Part Time employees. Plan pays from the 8th day to the 90th day of an approved disability with a maximum weekly benefit of $600 for full time, $540 for flex full time, and $450 for reg part time employees.
All enrollments in this plan are for a minimum 24 month period. Each covered annual period begins on January 1st. Eligible employees may enroll one time per year during the open enrollment period which occurs during the last quarter of each calendar year.
Employees have the choice of paying for this benefit with Earned Time hours or through payroll deduction of cash premiums each pay period
Long Term Disability
Eligibility: Full Time, Flexible Full Time, Regular Part Time, and Part Time employees. This benefit is paid entirely by the hospital. 60% of base wages after 90 days of disability. Insurance takes effect on the first of the month following three months of employment.
Healthy U-- The NMC Employee Wellness Program
Eligibility: Full Time, Flexible Full Time, Regular Part Time, and Part Time employees.
Eligible employees can receive financial incentives (in addition to better health!) for participating in the Healthy U Program. Five incentives are available each year for participation in the following activities: health risk appraisal; being tobacco free; completion of preventive screenings; providing evidence of physical activity; NMC employee wellness programs. Incentives are paid as taxable income in the employee’s paycheck.
